A recent and concerning study published in The Lancet Regional Health Southeast Asia has revealed an alarming public health challenge facing India: nearly 38.9% of Indian adults may be silently battling metabolic-dysfunction-associated steatotic liver disease (MASLD). Formerly known as non-alcoholic fatty liver disease (NAFLD), MASLD is a condition where excessive fat accumulates in the liver, potentially leading to inflammation (hepatitis) and, if left untreated, severe liver damage including cirrhosis.
The extensive Phenome India-CSIR Health Cohort, which involved over 7,700 adults from 37 laboratories across 27 Indian cities, painted a stark picture, challenging long-held beliefs about liver health. For years, fatty liver disease was predominantly associated with obesity and type 2 diabetes. However, medical experts are now witnessing a surprising shift: MASLD is increasingly being diagnosed in individuals who maintain a normal body weight and exhibit seemingly normal routine medical reports. This phenomenon has given rise to the term "lean fatty liver."
A leading gastroenterologist highlights the common misconception: "Many individuals assume that if their weight is normal and routine tests like liver enzymes, blood sugar, and cholesterol are within normal limits, their liver must be healthy. Unfortunately, this is not always the case. A normal outward appearance doesn't automatically equate to a healthy internal metabolism or a healthy liver."
The prevalence of MASLD in seemingly healthy individuals points to a complex interplay of factors beyond traditional risk indicators:
The silent nature of MASLD is its most concerning aspect. In its early stages, the disease often presents no discernible symptoms, and routine liver enzyme tests may remain normal for years. Consequently, many patients receive an incidental diagnosis during an ultrasound or FibroScan conducted for unrelated reasons. If left unaddressed, fatty liver can inexorably progress to severe inflammation, liver scarring (fibrosis), and eventually, irreversible cirrhosis – a life-threatening condition.
The critical takeaway for the public is unambiguous: liver health extends far beyond mere body weight. Embracing a holistic and preventive lifestyle is essential for everyone, irrespective of their current weight or perceived health status. Experts strongly recommend the following strategies:
Medical professionals advise, "If you experience persistent fatigue, unexplained abdominal discomfort, have a family history of liver disease, or lead a predominantly sedentary lifestyle, it is highly advisable to consult a gastroenterologist or hepatologist. Early diagnosis coupled with diligent lifestyle modifications can often reverse fatty liver and effectively prevent long-term liver damage. A truly healthy liver is a reflection of consistent healthy choices, not merely a healthy outward appearance."
